1. By signing this application, I authorize all previous employers or persons named in my application, to furnish BCH Gaming Reno, LLC., ("the Company”) all information they may have about me, including, but not limited to my work habits, skills, character, and the reasons for my leaving. I hereby release them and the Company from all liability for damages that may occur from the release of such information. I authorize the Company to conduct whatever background investigation it deems appropriate, and I understand that any false statement by me, or failure to disclose fully all information requested above my signature on this form, may disqualify this application from consideration, may disqualify me for employment, or, if employed, may result in my dismissal.
a. In the event of my employment with the Company, such will be completely voluntary, at-will, for an indefinite term, and may be terminated by me, or the Company, at any time, and for any or no reason not prohibited by law, and with or without prior notice. All employment is continued on that basis, and I understand that no supervisor, manager, or executive of the Company, other than the CEO, has authority to alter the foregoing. I understand that if I am employed by the Company, my status as an at-will employee may not be changed or modified by any practice or procedure of the Company or in the industry, or by any policy manual or other document issued by the Company except by a written employment contract executed by the CEO and myself, which pertains solely to my employment, and which specifically revokes the employment at-will relationship.
b. A clean drug and/or alcohol screening is a requirement for employment, and I hereby authorize reasonable use of my drug and alcohol screening results – both as part of this application process, and, if I am accepted for employment, as per the Company’s Substance Abuse Policy, to make decisions affecting my employment application and/or continued employment, as applicable.
c. If I am not hired within 90 (ninety) days of the date hereon, this application will then become inactive, and I must formally reapply if I wish to be considered for employment.
d. If I am seeking a position that might involve driving on Company business or driving a Company vehicle, before an offer of employment is valid I must provide proof that my driving record, verified by a current motor vehicle record (“MVR”), meets the requirements of both the Company’s Driver Safety Policy and the Company’s insurance company.
e. An investigation and/or Consumer Report which may include information as to my character, general reputation, personal characteristics, mode of living or credit worthiness may be requested by the Company in connection with my employment or post employment activities up to and including the State Department of Gaming, if applicable to the position. I hereby authorize the Company to obtain and review such reports, and further understand that upon request, I may obtain additional information about the investigation and/or report pursuant to the requirements of the Fair Credit Reporting Act.
a. To meet employment requirements of the United States Commission on Immigration Services and to submit appropriate documentation to satisfy the requirements for completing USCIS Form I-9, if requested.
b. To sign a confidentiality agreement if requested to do so.
c. To abide by all policies, rules, regulations, and performance standards of the Company, and as such may be amended from time to time at the discretion of the Company.
d. In the ordinary course of business, the Company can and may change my pay and benefits, or other conditions of employment at any time as laws, employee, and/or business needs change.
e. To meet minimum age requirements of applicable laws and submit proof of true age, if required.
f. That the Company may obtain from any applicable government or other office, at any time the Company so desires, my current motor vehicle record (“MVR”), including any personal information that might be included there.
g. That I may be photographed or videotaped as part of marketing activities for the property. I release any and all parties from royalties and by signing this application agree to allow my likeness to be used in commercials and/or still photos for the purposes of company related communication advertising or marketing in any form.
h. That although management makes an effort to accommodate individual preferences, business needs will make the following conditions mandatory; overtime, shift work, a rotating work schedule, or a work schedule other than Monday through Friday. I understand and accept such scheduling requirements.

b. No officer, agent or employee of the Company has made any representations or promises to me concerning length, duration, kind, or nature of employment or compensation which may be offered to me, and I have not and shall not base any decision to relocate upon any representation of such a nature.
c. I have read and thoroughly reviewed this completed application to the best of my knowledge, and understand that any misrepresentation or omission of facts is grounds for refusal to hire, rescission of offer, or, if hired, dismissal.
