Win/Loss Request

In order for Boomtown Reno to release information regarding your tracked play you must complete and submit a signed Win/Loss Request Form.

Once the Request Form is completed you can submit the form for processing in one of three ways:

1. By mail to address listed on form
2. By facsimile to number listed on form
3. In-person

 to download our Win/Loss Request Form.

Please allow 2-4 weeks for request processing. 2007 tax year requests will be processed beginning in February 2008.
